Request to Prevent Disclosure of Directory Information

Use this form to request that Lesley University withhold your Directory Information from third parties. This form must be received by the Office of the University Registrar prior to the close of the Add/Drop period for a given semester. Your request remains in effect until you submit written notification to revoke it.
