The Office of Operations and Facilities Services manages the issuance and tracking of keys for faculty and staff across Lesley University. Who This Applies To
This process applies to all faculty and staff requesting keys for spaces where they work or teach. It does not cover keys for residential students living in the dormitories. Those are handled by the Residence Life office.

How to Request a Key
All key and ID card requests must be submitted using the official forms below. Hiring managers are encouraged to submit requests for new employees before their start date.
- Facilities Key Request Form: Submit this form to request a key
- Public Safety ID Request Form: Submit this form to request an ID card
After submitting, your request will be routed to your immediate supervisor or manager for approval. Approvals are sent via Microsoft Forms. If your direct supervisor isn't listed as an approver, escalate to the next level up in your department hierarchy.
Urgent Requests
If there's an immediate need and your approver isn't available, you can submit a request through the work order system:

You can also call Fressia Maldonado at (617) 349-8881, Monday through Friday, 8:30 AM to 5:00 PM. Please note that a completed form is still required even for urgent situations, and this method should be used only when necessary.
Key Types
The level of key you receive depends on the type of access required. When submitting your request, please include details about the need for the key and the location(s) where it will be used.
Key Type | Description | Approval Required From |
Level 1 (Master Key) | Master suites, mechanical rooms, IT rooms, building masters, and exterior doors | Chief Operating Officer, Chief Human Resources Officer, or AVP Operations |
Level 2 (Operating Key) | Office doors, work areas, or suites | Department Head |
Incidental Key | Keys specific to an individual's workspace (file cabinets, desks, safes, etc.) | Department Head |
Keys and spare copies are also stored in secure lockboxes at Public Safety desks on each campus: University Hall (Porter Campus), McKenna Hall (Doble Campus), and Burnham Hall (South Campus).
Lost or Stolen Keys
Lost or stolen keys must be documented and reported. Department heads are responsible for reviewing lost key reports and working with Facilities Services and Public Safety to determine whether a rekey of the affected area is necessary. Please be aware of the following charges:
- Lost or stolen key: $50
- Lost ID card: $25
